I'm trying to get to level 9.2 for the beep test "shuttle run", at the moment I can only manage level 4.0..even then I'm nearly dying.
Can anyone tell me the best type of training I should be doing to improve strength in this area? I have a treadmill at home, maybe I should be doing some specific cardio on that?

why 9.2?
you're probaby better off doing repeated sprints...say something 35m sprints in a certain time...maybe time your best 25m srint time from a stop start (the beep test distance is 25m isn't it?)...then add 5 - 10% of that time onto it and that's the time you need to do your 35m sprints in...stop once you can't do 35m in that time increasing each session
if you can't get past 4 which is barely even jogging speed then you need basic work capacity improvement (farmer's walks, stair running, wheel barrow walks, tire flipping etc exercises that require to move something heavy over and over, for distance or time)
