A few points of clarification please 
.
1. What will the information be used for?
2. The reason listed on the survey for collecting it is a bit unclear.
3. The survey doesn't account for people doing outside and / or home based excerise, so unless the survey is specifically relating to gyms in some way, as opposed to peoples perceptions of themselves and exercise in general, your survey could look a bit discriminatory.
