Just take everything as a suggestion, its all trial and error i found what works for me may not work for you. You always try you push it till you cant anymore, if you think you can do one more rep, you do two reps. A good partner or trainer will push you if you want to be.

A catastrophic injury can prevent you from completing a lift. If you are benching and you have a catastrophic injury a single spotter cannot help you. A Smith Machine will suffice in that case, but it prevents you from performing the very bottom of the lift.
